We have completed the process of transitioning from Homelink to HMIS. All documentation should be completed and uploaded to/entered in HMIS.

Before you can do anything, you need to fill out the Consent Form.
Print out the form and hold on to a hard copy of all your consent forms (you will need to be able to produce the original for auditing purposes). Upload a copy into HMIS.

Once you have obtained the following paperwork: Birth certificate, I.D., and social security card, you are ready to fill out the Match Initiation Form for your client:

4. Ensure people have the opportunity to be connected to support services.

Print the form out, fill it out, and submit it to jessica.ivey@nashville.gov.

This form will help us ensure that a person is connected to support services and case management at the level they need. Even if your person does not want to be connected to support services, fill out the form.
