We have completed the process of transitioning from Homelink to HMIS. All documentation should be completed and uploaded to/entered in HMIS.

Before you can do anything, you need to fill out the Consent Form.
Print out the form and hold on to a hard copy of all your consent forms (you will need to be able to produce the original for auditing purposes). Upload a copy into HMIS.

3. Match Readiness
Once you have obtained the following paperwork: Birth certificate, I.D., and social security card, you are ready to fill out the Match Initiation Form for your client:

4. Ensure people have the opportunity to be connected to support services.
Critical Time Intervention Referral (filled out by Housing Navigator)
This form is only to be completed for those who are not eligible for other support services and need ongoing support services post-housing. The client must consent to these services prior to the application being submitted.
Print the form out, fill it out, and upload it to the Client Profile in HMIS, using the CES EDA Mode. Once you have uploaded the form, e-mail jessica.ivey@nashville.gov to inform her that a CTI Referral has been uploaded. Include the HMIS ID in your e-mail.
NOTE: If you do not e-mail Jessica Ivey, she will NOT know that a referral form has been uploaded, and will NOT be able to process your referral.

5. Give people an overview of what to expect as a new tenant.
