MHC Connector Card
To request a Connector Card for a client, please fill out the following form:
The form will go directly to Metropolitan Homelessness Commission staff. Please do not promise the person a Connector Card. Homelessness Commission staff will get back with you with any follow-up questions and determine eligibility of the request.
The Connector Card Code of Conduct must be signed by the client in order to receive a Connector Card.
The Connector Card Service Checklist will be used to track services accessed through the Connector Card program.
Refer to the Connector Card Training PowerPoint and the official Connector Card Policy document to answer any questions you have about the process.
