MHC Connector Card
To request a Connector Card for a client, please fill out the following form:
The form will go directly to Metro Homeless Impact Division staff. Please do not promise the person a Connector Card. Homeless Impact Division staff will get back with you with any follow-up questions and determine eligibility of the request.
The Connector Card Code of Conduct must be signed by the client in order to receive a Connector Card.
The Connector Card Service Checklist will be used to track services accessed through the Connector Card program.
Refer to the Connector Card Training PowerPoint and the official Connector Card Policy document to answer any questions you have about the process.
***NEW INFORMATION: After the client has been participating in the program for one year, Housing Navigators must complete the Annual Review. This is required for all clients who still have active Connector Cards at the one-year mark. This form is also used to determine whether a client is eligible to receive a three-month extension on their Connector Card. Please read all the instructions on the form before completing it.
Note: If your client has been permanently housed, but you have not completed a CES exit in HMIS, you will not be able to complete the Annual Review until you do so.
